We are excited to announce that in July, PKB is live in Imperial College Health Care Trust’s Renal Dialysis department (St Charles service). The deployment gives patients access to their pathology/radiology results, outpatient appointments and ADTs.
We are also now active across three sites in Wales. All three deployments are providing young adult patients with a communication tool and resources to improve their transition from paediatric diabetes services to adult diabetes services.
The sites are:
- Aneurin Bevan Local Health Board Young Adult Diabetes Team at Nevill Hall Hospital in Abergavenny
- Abertawe Bro Morgannwg University Local health board Young Adult Diabetes Team at Singleton Hospital
- Betsi Cadwaladr University University Health Board Paediatric Diabetes Team at Glan Clywd Hospital
