- University Hospitals Sussex NHS Foundation Trust
- PRH Physiotherapy Team – The team have started using PKB (named locally as My Health and Care Record) to create care plans for their patients. They are using 5 different care plan templates to share information and advice following Physiotherapy assessment which can be accessed electronically. Since going live in November the team have created and shared almost 500 care plans.
- Torbay and South Devon NHS Foundation Trust
- Torbay and Bristol Shared Care Gastroenterology – The team are working with a small cohort of paediatric patients and their carers to use PKB for the management of their care.
Along with the UK Kidney Association (UKKA) we are continuing to roll out PKB to renal units across the country to provide renal patients with a more secure and improved functionality platform giving patients more data and more tools to manage their condition. Read more about it here.
