In June we are introducing the first version of our Population Health Management (PHM) Engine. This tool is designed to benefit both healthcare providers and patients. It can be used to identify specific patient cohorts for strategic clinical planning and to deliver automated, personalised guidance directly to patients.
Any adult in England can now add to their PKB record a copy of the data the GP has released to the patient. We are proud to have done this across all of England’s GP surgeries. The integration gives patients more control over their health information and better sharing between healthcare providers.
