We’re pleased to share with you that more teams joined the Patients Know Best (PKB) community in August and have started using PKB with their patients!
- Neath Port Talbot Hospital (part of Swansea Bay University Health Board)
- Dermatology General – to reduce in-person appointments when patients’ blood test results need to be checked before a repeat prescription can be given
- Morriston Hospital (part of Swansea Bay University Health Board)
- Neurology: Myasthenia Gravis – to use digital care plans with patients and monitor their symptoms
- Hull University Teaching Hospitals NHS Trust
- Multiple Sclerosis Blood Monitoring Team – to use questionnaires to remind patients to have their bloods done before their appointments
- East Sussex Healthcare NHS Trust
- Trauma Team – using PKB and phone support with a limited number of Major Trauma patients to support the Traumatic Acute Brain Injury (ABI) patients when being discharged home over the weekend
- More GPs in a number of practices within Central London CCG situated in North West London are now accessing the Care Information Exchange (powered by PKB)
