Chelsea and Westminster Hospital NHS Foundation Trust – Specialist Clubfoot Clinic: as part of the Care Information Exchange in North West London this team are using care plans and sleep surveys for children going through treatment for clubfoot (“Ponseti method”) casting phase, maintenance phase with boots and bars.
CORADU – Collaboration On Rapid AntiDepressant Use: two teams in this project are now using PKB. One Research Registry Team and one local team based in Oxford Health Foundation Trust with a plan to roll out to further local teams nationally and internationally. The project is looking at the use of Ketamine for treatment resistant depression. Monitoring patient progress in PKB through care plans and consultation forms both at the point of referral and throughout treatment.
West Middlesex – Renal Dialysis Team: as part of the Care Information Exchange in North West London this team is providing patients access to their pathology and radiology results as well as outpatient appointments.
Cardiff & Vale University Health Board – Paediatric Cochlear Implant Team: encouraging remote management and monitoring post cochlear implant surgery by providing paediatric patients and their carers with access to a list of dedicated resources via the library and two care plans. One care plan provides pre and post operative care information and the second plan includes a range of questions for the patient/ patient’s carer to complete to assess and monitor the patients’ capacity to hear.
