Sites Live April 2018

We are excited to announce the following sites are now active this April!

Imperial College Hospital Trust –  Type 1 Diabetes Enhanced Services: as part of the Care Information Exchange in North West London this team is providing patients access to their pathology and radiology results as well as outpatient appointments. • Phase 1 – patients will be asked to complete pre-clinic questionnaires. • Phase 2 – may include messaging, video consultations and patient recorded measurements.

Royal Brompton – Cardiothoracic (Lung and Heart transplant) Team:  as part of the Care Information Exchange in North West London two sub groups of Cardiothoracic (Lung and Heart transplant) patients are able to use discussions to communicate with their team, access their medications, patient education and support materials and a patient quiz which will be sent out via consultations.

Central and North West London – Major Trauma: this project rolls out access to the Care Information Exchange in North West London and helps with use of the Major Trauma Patient care pathways within North West London’s specially commissioned Major Trauma Network. The hospitals in the network are St Mary’s Hospital, Chelsea and Westminster Hospital, Ealing Hospital, Hillingdon Hospital, Northwick Park Hospital, Watford Hospital and West Middlesex Hospital. The Care Information Exchange is currently being used in the St Mary’s trauma unit where resource materials and rehabilitation prescriptions are being made available to patients via the platform. The next stage is to extend this to the wider North West London  Major Trauma Network. Health/care professionals will be able to view rehabilitation prescriptions, radiology reports, labs and outpatient appointments for patients who have been repatriated to the trauma unit from St Mary’s. The main CIE functionalities being used include discussions (rehabilitation prescriptions), library, laboratory, imaging and appointments.
