Latest Live Sites – January 2022

We’re pleased to share that we now have more new teams joining the Patients Know Best (PKB) community and using PKB with their patients!  Read more about the teams and how they’re using the platform to improve patient care.

  • The Advocacy Project
    • The team is using PKB to share personal health budgets with their service users.
  • GP practices across the City & Hackney GP Confederation in North East London are starting to switch on their feeds to share primary care data with their patients.

The organisations above, known as the City and Hackney collaborative, were hugely successful at last year’s 2021 Health Service Journal awards for their introduction and use of Patients Know Best alongside their wider programme of work. The recognition for their Digital Recovery Platform for Severe Mental Illness resulted in the amazing accolade of a win, two highly commended and a finalist position for their cross sector programme of work.  You can read more about it here

The Trust went live with the Care Information Exchange (which is powered by PKB)  in mid December with appointments, encounter events, and radiology reports. This is the final Acute Trust across North West London to go live and contribute to the CIE programme, joining Imperial College Healthcare NHS Trust, Chelsea and Westminster NHS Foundation Trust and The Hillingdon Hospital NHS Foundation Trust. 

  • Imperial College Healthcare NHS Trust 
    • The Pelvic Health Physiotherapy Team is using care plans and consultations with their patients to help guide their care. 
    • The PATCH (Providing Assessment and Treatment for Children at Home) Team is adding care plans to children’s records to help clinicians monitor the patient’s condition.
    • The Post Birth Contraception Team is using the consultations functionality with their post-partum mums to find out more about their post birth contraception choices and effectiveness.
  • Cambridge Community Services NHS Trust 
    • The Paediatric Epilepsy Team is using the following PKB functionality:
      • Messaging – to communicate with carers and patients to provide a two-way conversation and allow carers and patients the ability to message in with any concerns or questions.
      • Care plans
        • Seizure Diary Care Plan specifically for carers and patients to be able to describe their seizure activity.
        • All About Me Care Plan –  this is used for carers and patients to document all the information they are regularly asked about on admission to hospital removing  the need to answer the same questions repeatedly.
      • Videos – providing a secure area for videos to be uploaded of a patient seizure and allowing clinicians to easily access it when they need to.
      • Library – providing an extensive library of information for patients and carers, ensuring the most relevant information is available and is all in one place.

Along with the UK Kidney Association (UKKA) we are continuing to roll out PKB to renal units across the country to provide renal patients with a more secure and improved functionality platform giving patients more data and more tools to manage their condition.  Read more about it here.
