Development Update: The Patients Know Best (PKB) Population Health Management (PHM) Engine

In June we are introducing the first version of our Population Health Management (PHM) Engine. This tool is designed to benefit both healthcare providers and patients. It can be used to identify specific patient cohorts for strategic clinical planning and to deliver automated, personalised guidance directly to patients.

Why store your GP records in Patients Know Best?

In 1988 I was diagnosed with a rare immune deficiency. It is the single most important thing a doctor needs to understand about my health if they are treating me. My GP, whose general practice looked after me since I arrived in Cambridge in 1986, made a note of this at some point in 1998.

Development update: GP data for any adult in England

Any adult in England can now add to their PKB record a copy of the data the GP has released to the patient. We are proud to have done this across all of England’s GP surgeries. The integration gives patients more control over their health information and better sharing between healthcare providers. Better sharing improves care co-ordination, reduces the risk of medical errors, and ultimately leads to better health outcomes for patients.
