We are excited to announce that Patients Know Best has been shortlisted with our partners in two categories as Finalists for the EHI Awards. PKB was selected with our partner Quality Practice as finalists in the EHI Awards Best Use of Technology to Share Information with Patients and Carers category. We have also been selected with another partner Luton and Dunstable IBD team as finalists in the EHI Awards Best Use of IT to support Healthcare Business Efficiency category.
This is exciting for us as EHI awards is one of the largest industry awards in the UK. The winners of these categories will be announced at the EHI Awards 2014, in association with CGI.
We have been working with Quality Practice, featured in our blog post back in April, to develop a new QP Digital Care Plan Manager, which will help GPs meet Unplanned Admissions Direct Enhanced Services (DES) requirements. These aim to reduced unplanned admissions in hospitals by (a) creating a registry of the 2% of patients most at risk for unplanned admissions (b) improving access between patients and healthcare staff and (c) providing personal care plans for patients.
Our work with Luton and Dunstable Hospital IBD team, led by Dr. Matthew Johnson, is one of the first to pioneer patient portals for their inflammatory bowel disease patients. The patient portal is designed to help patients self-manage and enable the clinicians to monitor the patients in the community. The result from the case study showed that the IBD team were able to monitor patients in the community (using PKB as a communication portal with instantaneous guidance provided) and, when patients needed to be seen, they were accommodated in the clinics within 24 hours.
