LATEST LIVES SITES – MARCH 2022

We’re pleased to share with you the latest teams joining the Patients Know Best (PKB) community and using PKB with their patients!  Read more about the teams and how they’re using it.

  • Teams in Hywel Dda University Health Board
    • Within the Persistent Pain Pathway are three teams using care plans, consultations and library resources to support patients living with long term pain:
      • Persistent Pain : Osteoarthritis
      • Persistent Pain : Fibromyalgia
      • Persistent Pain : Pain Management Programme
    • Within Therapies are three teams
      • Prostate Cancer Prehab to Rehab – which will use the consultation feature to identify areas in which patients need support to allow the team to provide best care.
      • Occupational Therapy Rheumatology – which will use library resources to support patients.
      • Orthopaedic Prehab to Rehab – which will use the care plan feature to provide an 8 week prehabillitation programme to patients awaiting hip or knee replacements.
  • Teams in  Swansea Bay University Health Board:
    • Neurology – Neuro Rehabilitation NPT – Patients use the consultation feature to answer questions about their current treatment plan – benefits, outcomes, adherence – which allows clinicians to prescribe or amend new courses of treatment.
    • Neurology Morriston – Headache Service – the new Neurology (Headache Service) team in Swansea will be using a care plan to monitor patients’ headaches over a monthly period in order to prescribe them a new course of treatment without the need for face to face appointments.

Along with the UK Kidney Association (UKKA) we are continuing to roll out PKB to renal units across the country to provide renal patients with a more secure and improved functionality platform giving patients more data and more tools to manage their condition.  Read more about it here.
